The Philadelphia Phillies have placed day disabled list with a sports hernia.

The 35-year-old veteran last played for the Phillies on August 6 and was diagnosed with the injury two days later. This will be Polanco's second trip to the disabled list this season. He missed 19 games in July due to lower back inflammation.

In 89 games with the Phillies this season, Polanco is batting .275 with four home runs and 40 runs batted in.

To fill Polanco's spot on the 25-man roster, the Phillies selected the contract of right-handed pitcher Michael Schwimer from Triple-A Lehigh Valley.

Schwimer is 9-1 with 10 saves, 86 strikeouts and a 1.88 earned run average in 46 games with Lehigh Valley.

To make room for Schwimer on the 40-man roster, right-hander Joe Blanton was placed on the 60-day disabled list. Blanton has been sidelined since late April with right elbow inflammation.
