“Teen Mom 2” star Jenelle Evans is back in rehab, according to RadarOnline.
The gossip website reported Evans is seeking treatment for an addiction to heroin, according to a source.
“Everyone has been worried about Jenelle doing heroin, even though she was denying that for a long time,” the source said.
Evans has had a tumultuous few months, the source added, noting she recently separated from her husband, Courtland Rogers. The MTV star also reportedly recently got pregnant again but suffered a miscarriage.
The 21-year-old is now reportedly tying to get help. Her first stint in rehab-- for marijuana addiction-- was captured on “Teen Mom 2” back in 2011.
“She has been doing drugs, and she really wants to stop and live a clean life for herself and for her son, Jace,” the source told Radar.
Jace is currently being cared for by Evans' mother, Barbara.
The fourth season of “Teen Mom 2” premiered last week on MTV. Episodes of the show air Monday nights at 10 p.m. ET.
A rep for MTV told FOX 411 the network had no comment on reports Evans had entered rehab.
